Your stories and insights are essential to improving Ontario’s health care system.

Why do we want to hear your caring experience?

When you consider the health care system and how patients move between hospitals, community-based home care and long-term care, it doesn’t take long to realize that family caregivers are the glue that keeps the pieces together.

- The Change Foundation, Out of the Shadow, Into the Circle, 2015-2020 Strategic Plan

The Change Foundation and the Ontario Caregiver Coalition are committed to finding ways to enhance family caregivers’ interaction with the health care system in Ontario. As a result, we have partnered to create “The Caring Experience,” an outreach project to better understand, and ultimately improve, the experiences of people who provide care or support to those with episodic, chronic, acute, and/or frailty-related health issues.

The Caring Experience project is core to The Change Foundation’s current strategic plan and the underlying principle that “valuing and recognizing family caregivers as integral members of the health care team should be an urgent focus for our health system.” It is also consistent with the Ontario Caregiver Coalition’s goals of raising awareness of the value family caregivers add to the health care system, and advocating for improved, fair access to needed supports.

We are committed to the confidentiality of all those who participate in The Caring Experience project. We will be taking all the information we hear to create a report that highlights the themes of what we heard to help support positive change in Ontario’s health care system. No identifiable information will be used without explicit consent.
